Without seeing the photo, but reading the (google translated) responses of those who have, sounds like a bolt tear out failure. But i'd like to see the photo first.

Are you using also a pin to fix the carbon tube to the alu insert?
What is the hole in the upper side of the carbon tube?

I think you insert weak points making holes in the carbon tube.
If you can try to use only glue to fix the tube.
You can find bonding documentation on the website of Loctite.
For a good bonding it is important the surface preparation. a common error is to sand and clean.
the correct sequence is to clean the surfaces (use specific product or trichloroethylene, not other cleaners! they left impurity). then sand the surface, clean again.
